Patient Intake Form Summarizer

Turn a submitted intake form into a clean, structured summary. A webhook receives the form payload, an LLM extracts the key fields (demographics, chief complaint, medications, allergies) into JSON, the row is appended to a tracking spreadsheet, and the care team gets an email digest.
